Getting your CSOS digital certificate
form from the DEA:
Go to: http://www.deaecom.gov/applycert.html select the “DEA Enrollment” link and begin your sign-up process with the DEA.
Follow the instructions on the DEA website. The DEA usually issues your CSOS digital certificate within a few weeks.
Once you receive and download your certificate from the DEA, KeySource Medical’s CSOS system permits quick and easy ordering.
While waiting for receipt of your certificate, DEA paper form 222 can be accepted. Send completed form(s) to:
